As soon as the history is written into the “S” section of the SOAP note, the clinical assistant starts working: it puts today’s symptoms next to the vital signs, the underlying conditions, the medication in progress, the allergies, the lab results and whatever was recorded in previous visits, and hands the doctor a list of likely diagnoses — each with a confidence level and the evidence from this very record that produced it.

What path has this patient taken over time, and what did the treatments do?
Everything in the record lands on one shared time axis: measurements as trend lines, courses of medication as spans, and diagnoses, doctor’s orders and visits as events. Because they share an axis, reading down a column means “at that same moment” — so it is easy to see when a medication started and what the patient’s blood pressure, blood sugar or weight did afterwards.

All data is kept on infrastructure inside Iran. Access is controlled by centralised authentication, and each centre reaches only its own records.
No. AI output — the visit summary, the differential, suggested medication and tests — is all suggestion, and until the doctor approves or edits it, it is shown with an “AI suggestion” label.
Creating an account and starting work takes a few minutes. Migrating data from a previous system and training the reception team usually takes one to two working days.
The model is tuned on Persian clinical speech and knows the drug and diagnostic vocabulary. The transcript is always editable and reaches the doctor for approval before it goes into the record.
